What information do we gather and how is it used?

Outlook Health provides you with related tools and information to assist you in shopping for and obtaining products and services online. Most of all the tools and information are available for your benefit without the need to disclose any personally identifiable information. If you decide to request an application or online quote, we require you to submit minimal personal information. We collect information that includes your name, e-mail address, mailing address and telephone numbers.  A representative from an Outlook Health subsidiary will contact you to complete your online request by telephone so that additional information can be obtained.  When information is provided to Outlook Health or an Outlook Health subsidiary online, it is protected in the same way that all information provided to Outlook Health is protected. (See Security below)  Outlook Health will not sell, trade, lease or rent any personally identifiable information to any other companies or organizations. Outlook Health will not knowingly solicit data from children, and we will not knowingly market to children. Outlook Health may share aggregated statistical information such as website traffic or demographic information with its business partners; however, in no case will your personally identifiable information be disclosed. We retain your personal information for 30 days.  Outlook Health may send you e-mail communications to update you on website features or new benefits that may be of interest to you. A cookie is a piece of data stored on the user’s computer tied to information about the user. Usage of a cookie is in no way linked to any personally identifiable information while on our site. We use both session ID cookies and persistent cookies. For the session ID cookie, once users close the browser, the cookie simply terminates. Social Security Numbers

The Social Security number is an optional field that can be used to associate Outlook Health members with their accounts.  If this field is left blank, our system will automatically generate a membership identification number that the customer may be identified with.  Again, this field is entirely optional. When you enter your social security number on our enrollment form, we encrypt it using secure socket layer technology. (SSL)

Notification of Changes

If at some point in the future, there is a change to our information usage practices that affect your Personally Identifiable Information, we will notify you by email of the relevant changes. The email will inform you on how to opt-out of this information usage or you can opt-out by sending a message to the following email: communication@OutlookHealth.com. You should also check this posted privacy statement for changes.
